On March 23, 2005, BPs Refinery in Texas City had an explosion during the restart of their hydrocarbon isomerization unit with the subsequent fire killing 15 and injuring 180 people. This was the most serious refinery accident ever investigated by the US Chemical Safety and Hazard Investigation Board (CSB) (Figure 1). CSBs investigation report and video outlined the operational mistakes along with highlighting the challenges with their continuous level and switch measurement points.
