Abstract: The A.G.A. Plastic Materials Committee has developed a standard Plastic Failure Report Form. It provides a standard format for gathering necessary information for proper determination of the nature and the cause of a failure on the plastic pipe system. The paper presented here discusses the format of this form, its application and potential benefits to the user utility and the gas industry.
